paperwork we need to check and recheck.

You need:



A notarized copy of your Dr’s license – it cannot expire in the next year to be safe, it must also be apostilled.

A certified copy of your marriage certificate – it must have an apostille from the state it was issued.

Certified copies of your birth certificate and your husband’s. These also need to be apostilled.

A notarized letter from your tax assessor’s office (showing you own a home, the square footage, # of bedrooms, etc) this also must be apostilled.

A copy of your social worker’s license and the agency that you are using license. Both of these need to be apostilled.

A notarized Power of Attorney for your facilitator - also apostilled ( you may want to do the same thing for your husband if he wants to return early)

A criminal clearance from the state bureau of criminal apprehension for each of you- notarized and apostilled.

Copies of your passports with a single letter swearing that they are both true and unaltered – notarized and apostilled

Medical clearances from your dr with a letter swearing that they are both true and unaltered – notarized and apostilled

A notarized copy of your Dr’s license – it cannot expire in the next year to be safe, it must also be apostilled.

Employment verification letters (or a letter stating that you are a homemaker) notarized and apostilled.

A letter stating that you will follow the reporting obligations of Ukraine – notarized and apostilled

A COPY of your I-171 H with a notarized copy saying it is a true copy, notarized and apostilled.

A letter requesting an appointment for specific children (notarized and apostilled.)

An adoption petition letter (general request) notarized and apostilled.



Bring your ORIGINAL I-171H and 2 years worth of your taxes for the Embassy. Probably should have copies of all the above since you are working so hard to gather this information. Oh- also make sure that your Doctor answers “no abnormalities” under medical findings and “non-reactive/ negative” under lab results. I think that is all but you need to fax or scan copies to Victoria – she will tell you if something is amiss.

WE CAN'T THANK YOU ENOUGH. WE HAD TO HAVE PAPERS RENOTARIZED TODAY, THE OTHER ONE WOULD EXPIRE ON APRIL 2008. THAT WON'T WORK, NEEDS TO BE GOOD FOR A YEAR.
