Written request with justification singed by at least one if the prospection adoptive parents. (a short explanation or cover letter explainig the reasons and circumstances of the adoption.) This is where you describe the child.... age, gender, down syndrome, etc. in this letter, mention support you have for a child with ds and all of that. This should be appostilled as one document.
Short biography of prospective adopting parents: Basically it is the same as the first few pages of your home study.... But it has to be separate document from the home study. The social worker will read this, so think of it like you are writing to her. This should be appostilled as one document.
Home Study: needs appostilled
Prospective adoptive parents' Birth Certificates: Each should be appostilled seperately from the state in which you were born.
Marriage Certificate: Appostilled
Criminal Record: FBI BACKGROUND CHECKS for each parent: These each need to be appostilled separately. We attached a cover sheet and had it notarized and appostilled.
MEDICAL REPORTS: On general health condition. There is no specific form for this . We actually had our doctor write a letter on letterhead. It has to specifically mention HIV NEGATIVE, FREE FROM STD AND THAT YOU ARE MENTALLY FIT TO PARENT AN ADOPTED CHILD. NOTARIZED AND APPOSTILLED SEPERATELY.
EVIDENCE OF EMPLOYMENT::; INCOME, ASSETS, AND HOME. Make sure all of this mentioned in your home study . You also need an Empolyment Letter that specifically lists salary. Have the Employment Letter Notarized and Appostilled.
Evidence that prospective adoptive parents meet the U.S. Adoption Requirements. ( I 171H IS ACCEPTED):: I171H NEEDS TO BE NOTARIZED AND APPOSTILLED.
Evidence of U.S. citizenship( passport):: Copies of Passport. In black ink.We had these copied on the same page so we only had to have one notary and appostilled.
PHOTOGRAPHS::: SEND SEVERAL PHOTOS OF YOUR FAMILY, YOUR HOUSE, ETC. WITH A DESCRIPTION OF THE PHOTOS. WE SENT 12-OR MORE PHOTOS AND TOOK MORE WITH US.
